NYS Sheriffs' Association Yellow Dot Program

The Yellow Dot Program is a project of the New York State Sheriffs' Association. It is designed to alert and help first responders provide life-saving medical attention during emergencies. You simply fill out a personal information card which includes emergency contacts, physicians, preferred hospitals, medical conditions, medications, and allergies then place it in the instructed areas along with a yellow dot decal.
